Agreement Page Two

After pressing submit you will be taken to the payment page where you choose you course date if applicable, and the option to pay by Google Checkout, PayPal or cheque.

It’s important that all students joining a course have to complete the agreement below, even if a different person is paying for the course. There are two parts to this document:

Page one, the terms and conditions relating to your enrolment and your CIDBT’s responsibilities. Click here to view page one.

Page two, an Agreement Form which you need to complete below.

We will send you a copy of the agreement form at the same time that we send your paid invoice to you.

When you press the submit button you agree to abide by the content of the agreement (a digital version of signing an agreement with a pen). Fields with a Star* must be completed. Payment options are Google Checkout, PayPal or cheque.

I have read page one of the Enrolment Agreement, which I have read, understand and I agree to comply with all of its terms. I agree to comply with all CIDBT policies and rules contained therein.

I agree that no binding promises, representations or statements have been made to me by CIDBT or any employee of CIDBT regarding any aspect of the education and training I will receive from the CIDBT other than those in the Course Pack, Enrolment Agreement or Acceptance Letter.

I understand that programs are offered on-line and have published start and end dates.

I understand that I may be required to attend residential's that may be at some distance from my home and that I am responsible for expenses associated with travel to the workshops and living expenses while there, other than costs which are specified in the Course Pack as being included in enrolment payments.

Note: If you book a course that contains a workshop that part of your course is mandatory and you will be advised of the day or days when you should attend. Only extraordinary circumstances will be considered for a cancellation and none less than 4 weeks before the course workshop date(s). If you do not attend you will have to pay for that same workshop again for the next available date we are able to offer you to complete that part of your course.

I understand that I will receive a book list for each course and am responsible for purchasing these books and that costs of books, CDs, DVDs and other resources may vary from indicative figures provided.

I understand and agree that this Enrolment Agreement may not be modified without the written agreement of both CIDBT and myself.

I hereby certify that all information I provided in my application for admission or registration to the CIDBT is complete, accurate and up to date.

I have read the terms and conditions contained in this Enrolment Agreement and understand that submitting this form electronically constitutes my confirmation in lieu of a signature and that this agreement constitutes a binding contract upon written acceptance by the CIDBT.
